We've heard that using a chemo drug in a lower dosage (such as Xeloda or Cisplatin) in conjuction with radiation can increase the effectiveness of the radition. Would you recommend that we should do the radiation with an an extra agent and if yes which would you recommend? Also you recommended that we should do radiation first and then chemo unless there is a long delay before we can start radiation, in which case to do the chemo first. What is considered a long delay (considering that the surgery was on 28/jan/2010)?Once again, thank you .Uriel

First of all, thank you very, very much for your detailed answer.My wife has had surgery to remove the recurrent tumors. I would like to clarify if you recommend doing the radiation therapy before the chemo or doing the chemo first? I presume that by platinum based chemo you mean Cisplatin. Between Cisplatin alone or with Gemcitabine which would you choose?Please note that while it is easier for me to write in English, you may certainly answer in Hebrew.Once again, thank you.
My wife, Age 49, who is BRCA1 positive with a strong family history of breast cancer, had a Stage I, Grade 3, Triple Negative, breast cancer in her left breast in June 2008, and had a double mastectomy followed by dose dense chemotherapy (4 x AC every 2 weeks then 4 x Taxol every 2 weeks) finishing in November 2008. She did not have radiation therapy. In Jan 2010 she was found to have a regionally recurrent breast cancer in the left breast and had surgery to remove a tumor in the breast and an axillary lymph node. The biopsy revealed cancer in the breast tissue and in the fat tissue surrounding the removed lymph node (not in the lymph node itself.). A PET scan before the surgery did not detect any cancer (not even the one in the breast because it was small). The treatment protocol calls for Radiation but there is no protocol for continued treatment now regarding chemotherapy.
We did the TargetNow analysis with the following results which were categorized as Significant:

I'd be interested in hearing your opinion: 1) Should we do chemo, 2) If yes to chemo, then which chemo? 3) We've heard about Iexmpra, do you think this is relevant? 4) Are there any medication that are not in the health basket which you would recommend?Thank you very much.
