It is probably related to continued sperm production in the testicles with nowhere for the sperm to go. So they are trapped in the duct system. This causes the duct system to become congested and distended and causes pain. It would be best to be evaluated for this by a urologist. Sometimes, a pelvic CT scan is also indicated to rule out any other possible cause of pain.
